Documentation mistake 1: If it is clear to the physician, it will be clear to the coders.

Physicians may assume that coders can get what they need by, for example, looking at lab values or other test results. But according to national coding guidelines, coding staff cannot code from lab values, and they cannot diagnose. Coders need the narrative terms required to support key diagnoses.

Under the new MS-DRG system, the reality is that we need a lot more specific and detailed verbiage in the medical record to support correct coding.

Documentation mistake 2: Someone else will fill in the blanks for you.

Don't let physicians pass the buck when it comes to proper documentation in the medical record. Coders can't just pluck support for their codes from anyone's notes in the chart. For example, information regarding malnutrition must come from the physician (or a physician assistant or clinical nurse specialist) - not from a dietitian.

Documentation mistake 3: Physicians drag their feet when coders' queries come their way.

This is a mistake - but not necessarily the physicians'. How physicians respond to queries for more information depends in large part on how knowledgeable they are regarding the coding process - and health information managers are responsible for securing that buy-in.

Don't put your practice at risk by staying behind the updates. Get the recent coding news, or your practice's revenue will suffer.

NBA Lockout 2011 News - What Is Going to Happen?

As you may know as a fellow sports fan you would know many football fans across the world have been struggling over the recent NFL lockout. And now NBA fans across the world will struggle with the new lockout of the National Basketball Association otherwise known as the NBA. Some ESPN and other professional sports writers and reporters are predicting that the 2011 NBA lockout is going to be a lot worse than the 2011 NFL lockout. Some of the reporters even go so far to say that the NBA may not even have a season this year. I wish this was not true but we have to face the facts, it is all about the money and how much everyone gets paid.

Problem - First off what starts every lockout in the history of sports is the problem that somewhere someone is not getting paid enough throughout the ranks of the organization. It was up to me I would just have them play and just have a flat salary but that is not how the world works in every player is not of equal talent. And in this case the NBA 2011 lockout was primarily caused by the NBA officials and the NBPA (otherwise known as the national basketball players Association) So Let's cut to the chase. The Problem is that the collective bargaining agreement has expired and the NBA players in the commissioners cannot come to an agreement. A collective bargaining agreement is the profit share between the players and the owners and all of the rules and obligations they must comply with. This is the same problem that happened the NFL the players in the commissioners could not agree on the collective bargaining agreement just like the NBA. Although rumor has it that the NBA collective bargaining agreement problems will be a lot tougher to settle the NFL's disagreement.

I have many predictions about what will happen with the NBA lockout.

1. My first prediction is that the NBA lockout will get so bad that some of the players will not even want to play the NBA anymore. Some players may even want to play overseas to become national stars and I think some veterans that would have played in the NBA this year may retire so they don't have to put up with anything related to the lockout. The NBA could lose most of its stars and become not nearly as competitive as it was in recent years. Dwight Howard recently stated that there is a chance the lockout is so bad he may go to play in China. Many other NBA stars like Deron Williams and many other players.

2. Another one of my predictions is that they will come to an agreement just like the NFL but it will not be the greatest agreement and some players may even want to leave the NBA to play overseas. Although it is still possible they could come to a perfect agreement where both sides are making money and everybody is happy.

3. My last prediction is not the greatest of all predictions but is still possible to happen. The NBA lockout could become so bad that the NBA does not have a season for 1 to 2 years because they could never come to an agreement and eventually another lead could come over and take the NBA players from the NBA which could be a bad thing or a good thing it all depends on the situation.

Forex News - The Collapse of the EURO

The Wall Street Journal's coverage of the Greek dent crisis has focused less on the crisis itself, and more on the markets' reaction to it. With headlines like "Hedge Funds Try 'Career Trade' Against Euro" and "Speculators Bet Record Amount Against Euro For 4th Week" and "Europe Trouble, U.S. Opportunity" - among others - the WSJ has identified a collapse in the Euro (mainly against the Dollar) as one of the most prominent (and profitable!) strategies for exploiting the crisis.

The debt crisis has become self-fulfilling, not only for Greece, but also for the Euro. In other words, as perceptions abound that Greece is insolvent and the Euro is doomed, Greek bonds and the Euro have lost value, which only makes the crisis worse. It seems that speculators are taking advantage of this phenomenon by making large bets against the Euro. In fact, large is an understatement, as the net short positions against the Euro now total a record $12 Billion, according to the closely watched Commitment of Traders report.

Some analysts have taken such information at face value, noting that "The fact that the shorts got even shorter when they were already at extreme levels highlights just how negative the sentiment is toward euro." On the other hand, there is evidence (and some degree of admission) that large speculators are now acting in concert to bring down the value of the Euro. The WSJ reports mention private meeting between hedge funds managers and investment banks helping their clients bet against the Euro using derivatives. For those that are skeptical that speculators could really influence currency markets, consider that one man - George Soros - single-handedly forced a devaluation of the Pound in 1992, and made $1 Billion in the process. While the Euro is certainly bigger than the Pound ever was, there are more people watching it than ever, and when there is money to be made - hundreds of billions of dollars in this case - it isn't inconceivable that the Euro could suffer a similar fate.

Already, there is evidence that this strategy is working, as the Euro has fallen 10% in less than three months, which is unbelievable for a currency whose daily trading volume is estimated at $1.2 Trillion. In fact, one popular options trade is based on the the Euro falling to parity against the Dollar. Once unthinkable, such a possibility now faces odds of "only" 1 in 14 (based on options premiums), compared to 1 in 33 in November. On the one hand, it's frustrating to accept the market power that these speculators have. But emotion has no place in (Forex) trading, and standing in the way of momentum would be costly.

On the other hand, Euro fundamentals remain strong. To be sure, a currency is only as strong as its constituent parts, and the fact that a handful of EU member states have shaky finances certainly cannot be dismissed. At the same time, the fact that such currencies have no direct control over the Euro is just as important. Before the inception of the Euro, currency traders would be justifiably concerned that a country in a similar position to Greece would deliberately devalue its currency (by printing money) in order to devalue its debt and make it more manageable.

Now, this would be impossible, since the Euro is controlled by the European Central Bank, over which Greece has no power. The current crisis in Greece notwithstanding, "The European Central Bank's (ECB) resolve to maintain sound money is...important. This is especially true for the ECB, which has a single mandate-price stability-unrelated to fiscal problems." While there is legitimate concern that the ECB will be forced (or voluntarily) print more money to fund bailouts of bankrupt EU member states, this doesn't seem very likely, given the history of the ECB. Its monetary policy has always been quite conservative, and it's no wonder that the Euro has come to be seen as a viable alternative to the Dollar.

In my opinion, the decline in the Euro is mostly baseless, and if it were to continue, it wouldn't represent the prevailing of logic. Then again, logic is not exactly a word that I would apply to the Forex markets, now or ever.

Why The New Foreclosure Halt Is Great News For Investors

news of yet another foreclosure halt hit the headlines last week. Does this mean doom and gloom for real estate investors or could it be a fantastic present in disguise?

For those who missed the announcements last week, several mortgage giants have rolled out plans for a 'foreclosure moratorium' for the holidays. So what is it exactly, who will it help and perhaps most importantly, who will it hurt?

Fannie Mae, Freddie Mac, Chase and Wells Fargo all announced 'foreclosure moratoriums' for dates ranging between December 19th and January 2nd. However, what this really amounts to is a halt on actual evictions for those few days, during which these mortgage firms will forge ahead with the paperwork they need to begin the new year with a fresh flourish of evictions and foreclosures. So homeowners must avoid being lulled into a false sense of security which could have disastrous consequences.

Most other lenders including banking giant Bank of America have failed to follow suit, essentially increasing the speed at which they can carry out physical evictions and increasing the odds that their borrowers will be foreclosed on and given the boot in the week between Christmas Day and New Year's Eve.

This is certainly not an enviable position for any home owner to be in, though while it will certainly be sad to see families being thrown into the street many have no one to blame but themselves. Not because they weren't victims of the economy and bad government moves in the last couple of years but because they have chosen not to take positive action to help themselves. Unfortunately those who have failed to seek help will find themselves not only homeless but in many cases still owing on the mortgages on their old homes while those who are reaching out and are taking advantage of short sales are not just debt free but with fat checks in their pockets from lenders for relocation expenses.

While normally a halt in foreclosures may put investors in a bind due to a lack of inventory and deal flow, this current situation actually makes things easier for investors who are ready to move quickly.

Firstly this partial foreclosure halt means that it is easier for real estate investors to hone in on the most motivated sellers. This can be done by acquiring lead lists from a variety of sources including the credit bureaus and data compilers DMA Leads and Info USA.

Secondly it takes all of the work out of having to motivate sellers. It is an easy choice for them. Get kicked out in the street during the holidays and still be in debt up to the eyeballs or get out and possibly walk away with cash in hand to move somewhere else and actually get some nice gifts for those they love. All you have to do as an investor is to match your buyers to these sellers who are desperately in need of help and use flash funding to wrap them up before the end of the year.

Network Marketing and Internet Marketing - The Good News and the Bad News

Have you ever heard someone say, "I've got some good news, and I've got some bad news."?

Well, network marketing and Internet marketing fall into that category. For this article, by the way, I may use the two terms somewhat interchangeably as most network marketing programs are done online to some extent these days, and most Internet marketing programs are network marketing programs at their core.

Whichever one, where the problem arises is that the good news and the bad news is the same news. Whether it's good or bad in someone's mind depends on their particular perception and point of view.

The good AND bad news about a home based network marketing, or Internet marketing, business is that it's easy! This seems like a good thing, but can lead to bad things.

First, I made the statement above because I have worked a lot of jobs at pick and shovel level and at executive level, and several in between. Secondly, I have participated in quite a few network marketing and Internet marketing opportunities, and failed miserably in all. Then one day, the light came on, and I realized how easy all this marketing stuff really is! I am now successful with my home based network marketing business, which I mainly market online, and would like to share a little of what I have learned with you.

I have spoken to successful Internet and network marketers, including a couple of millionaires, and they seemed to be a little dazed at the fact that money was flowing in without them having to do much of anything.

The problem is that many of both types of marketers use the ease of Internet and network marketing as the bait to attract recruits into their programs, without being able or willing to explain the facts of marketing one of these businesses to their new recruits.

Let's examine this.

There's Fred and there's Sandra.

Fred sees a bunch of ads and job opportunities, but the one that catches his eye and lures him in is the one which offers him a huge income with little or no effort on his part. The business sounds good to him, but odds are, once whoever recruits him gets his money, Fred will never hear from his upline again. At least we can say that network marketing was easy for that guy, huh?

Sandra, on the other hand, is not looking for any get-rich-quick-scheme. She is honestly willing to invest some of her time to acquire a sweat equity in a home based Internet business, let's say. She understands about train-up times and learning curves, and she is willing to study, pay her dues, and take her lumps. She realizes that she needs to begin with an honest reliable business, offering a good product or service, with a decent business opportunity attached. She does her homework, makes her mistakes, and then.....

She believes that with time and experience it will get better.

She's right, by the way.

Be honest now. Who is most likely to succeed in life, work, relationships, anything?

You know what?

I think that I like Sandra and respect her opinion on things. If she's talking, I'm probably going to listen.

Fred, on the other hand, will probably turn me off almost as soon as he opens his mouth.

Okay, here's a POP QUIZ!

Since network marketing, whether done online or face to face, is about one person telling another person about a product or opportunity; and, since a lot of this transfer of information depends on trust and sincerity, who do you think is going to be the best network marketer?

Fred or Sandra?

You know, in the world of network marketing, the best marketer is not always the person with all the facts and figures at hand, although that certainly does help.

Simply put, the best network marketers, and the best online marketers, i.e. Internet marketers, are people who network.

I bet Sandra has a lot of friends who she shares time and thoughts with. Fred, on the other hand, will probably tell himself he's going to do something, but at best, he's probably going to sit in a room all day studying facts and figures, but if he isn't on the street talking to people, or answering the phone as they call in...he isn't selling anything or signing anybody up.

Take another look at Fred and Sandra.

Even if neither makes it big in network marketing, who is going to gain the most from the experience?

Why, Sandra, of course!

Okey, that's all good news for Sandra, so what's the bad news in the title?

After Fred fails, he is going to feel like he's been lied to, ripped off, conned! What's he going to do about it? He's possibly going to tell any one he can that such-and-such a company, or even network marketing, and Internet marketing, in general, is a real rip-off.

Now he's going to do some network marketing with a vengeance! He's motivated! They LIED to him! STOLE his money! He is emotionally involved and believes his story, and wants to share every little detail with anyone who will listen!

Why couldn't he do that as a distributor?

Well, he could have!

Anybody could have.

That's why network marketing is so easy!

And that's the good news.

All you have to do is tell people about your product, service, or opportunity. If you really do believe what you are saying, if the product is a good product, and the company is a reputable business, why shouldn't you be happy to tell people that they can get soap, cellular phones, plastic ware, insurance, vitamins, or weight loss products from you? If that person doesn't want to buy from you, fine! Go tell somebody else. Sooner or later, somebody will buy something.

That's the start of your customer base. Once you get a sale, or a recruit, go out and do the same thing again. It you did it once, you will get another customer. Keep it up and you will see a regular income appear. Just don't give up!

Once you have customers, treat them right and they will possibly order from you again. If they like the product and the way they are treated, they will possibly send their co-worker or sister to you. That's network marketing which will work for you, and you don't even have to present a sales pitch to them. Their sibling or co-worker did it for you.

You know what...?

If they send their sister AND a co-worker, they might be pretty good at this network marketing thing. That could be the start of a marketing network, couldn't it?

Finally The Good News About Accutane And iPledge

In March, the Food and Drug Administration (FDA) approved a stringent new program known as iPledge to monitor the distribution of Accutane. Accutane is a drug that treats a serious form of acne known as nodular acne.

This program was implemented because of concerns that arose about the correlation between the use of Accutane and serious birth defects occurring in the unborn children of women using the drug. Some doctors are have voiced out that Accutane is not safe for women who are pregnant or wants to become pregnant.

iPledge requires that all users, prescribers and distributors of the drug must register with iPledge before they can receive or administer treatment. Female patients of childbearing age must pledge to use two forms of birth control for the month before treatment, while they are on the drug, and the month after treatment ends. Additionally, female patients are required to test negative to two pregnancy tests before they can be given the drug, and must also agree to take a pregnancy test before each refill.

Besides causing birth defects, Accutane has been thought to be linked to suicide and depression in some people. However, so far there is no concrete evidence to determine whether Accutane could have caused these tendencies to occur.

The FDA also issued warnings that there are other serious side effects of Accutane, including causing the amount of lipids and cholesterol in the blood to rise, and problems with the abdomen, brain, blood sugar, hearing and vision, as well as decreasing the patient's red and white blood cell count.

Still, for patients who have struggled with nodular acne, Accutane may appear to be a miracle cure, after all other treatments have failed.

However, getting a hold of the medication may prove to be more difficult now with iPleadge. According to American Academy of Dermatology Association, many dermatologists had experienced problems with the iPledge system.

Despite the barriers, it may be good for consumers now that Accutane is closely regulated. If there are potential problems with Accutane, then the government, in this case the FDA should do everything they can to keep them from happening. After all, this is actually an optional drug. There are many other methods to treat acne problems. Some of them can be very affordable and natural too.

The most important thing is that having a child with birth defects is a serious thing compared to acne and cannot be undone. We need to take responsibility to prevent this. Consider very carefully when using Accutane.

Chocolate: Good News or Bad News? You Decide!

What puts chocolate on the list of health enhancing, anti-aging foods is its' high content of powerful antioxidant compounds called flavonoids. Other flavonoid-rich foods are tea, berries, grapes, apples and many of the other color filled fruits and vegetables.

The flavonoids in chocolate are called flavonols and have been shown to be helpful in lowering blood pressure, diminishing fatty plaques in arteries, and lowering clot formation by making blood platelets not so sticky. Most people know that the darker the chocolate, the higher the flavonol content.

The brain lifting compounds in chocolate are theobromine, caffeine, tyramine, and phenylethylamine (PEA). Additionally, tryptophan and anandamide are soothing and relaxing to the brain and promote the feeling of well-being. How can you not love chocolate when it gives you a dose of energy, clears your foggy brain, soothes your anxiety and dampens your pain all the while keeping your healthy cells healthy and blood flowing smoothly for fewer negative cardiovascular events?

Well, that's the good news-and now, the bad news.....

Even though there have been 2 large epidemiological studies showing the benefits of chocolate consumption specifically on lowering carotid artery plaques, blood pressure and other vascular problems, these studies were not well designed for clear and specific correlation about cause and effect from chocolate. Other flavonoid rich foods were being consumed simultaneously and chocolate contains additional health benefitting nutrients that obviously were not isolated from the flavonols.

Because chocolate has such a brain energizing effect, individuals with any chronic fatigue syndrome or adrenal exhaustion will be particularly drawn to consuming it, but cocoa, in addition to other caffeine containing stimulants should actually be avoided in this case. The same PEA in chocolate that lifts brain energy also creates a transient rise in blood pressure and blood sugar which is followed by a not good for you low blood sugar episode. If you have any kind of persistent or recurring fatigue issues, you need to support and nourish your brain to allow it to heal, avoid high glycemic reaction foods like chocolate and take in more energy sustaining proteins. Other conditions that can involve poor brain endurance and fatigue and should omit chocolate include Multiple Sclerosis, Parkinson's Disease, Closed Head Injuries and even Post Polio Syndrome.

Chocolate has been placed on the list of offending foods by many different researchers involved with migraines headaches, irritable bowel syndrome, allergies, joint degeneration, skin problems such as rosacea and mood disorders. Chocolate is also high in oxalates and most people who have suffered from kidney stones should be on a low oxalate diet which means no chocolate. Oxalates are also famous for binding calcium which prevents its absorption and a diet high in oxalates is not recommended for anyone trying to maintain or improve their bone density. If you have any bladder irritation or particularly have been diagnosed with Interstitial Cystitis (IC), chocolate should definitely be off of your grocery list. Anecdotally, there are groups of people who comment that chocolate weakens their immune system. They notice that they get sick soon after consuming even a small amount of it.

Despite chocolate truly having a high ORAC value (high antioxidant power), this healthy cell preserving ability is greatly diminished from processing, which is why it is best to eat high percentage pure dark chocolate and even better cacao in its raw form. Most of the chocolate consumed in the US is highly processed and can even include health stripping preservatives. It is pretty clear that chocolate does contain compounds that are health promoting, even the pharmaceutical companies are trying to isolate them, however, any benefits from flavonoids are totally drowned out when chocolate comes in the form of cake, caramel, ice cream, corn syrup and gooey candy bars. These foods do not count as healthy chocolate products.

So, you can hear the good part about chocolate, it is real, however, I'm hoping you can hear the negative aspects about chocolate as well and that it may not be a healthy food choice for you depending on the status of your health.

Still, Indiana's connection to the earth has made environmental concerns and recycling-including auto recycling and salvage-a growing feature of life in the Hoosier State. An update on the latest environmental and recycling news in Indiana follows.

Recycling Facility Gets Big Expansion

North Vernon, Indiana is a place you may not have heard of, but which is one of the epicenters of the growing environmental industry. Home to the world's largest recycling facility for plastic bags, owned by the largest manufacturer of the device loathed by environmentalists across the planet, its proprietor has announced plans to double capacity.

Hilex Poly aims to be the leader in recycled bag production with its North Vernon facility (which is one of the community's leading employers, now expected to hire more for the expansion). It has pioneered a program called Bag-2-Bag which reverses the distribution process, allowing grocery customers to return their bags to the store to be shipped back where they came from and sent to Indiana to make new bags. The company hopes to someday sell only bags recycled from old ones, a closed loop theory enticing to environmentalists.

Federal Funds Improve Environmental Infrastructure

Northwest Indiana is home to a federally protected lakeshore sand dune range beloved by those who frequent its recreational opportunities. With new funds appropriated by Congress for preservation and improvement of the infrastructure at the Indiana Dunes National Lakeshore, those who have worked to protect the ecologically unique and precarious area are inspired for the future.

Also involved in the environmental appropriation was almost a million dollars for improvement of a water reclamation facility in Portage, Indiana. As population increases in this part of Indiana, the older facilities were being seriously tested and environmentalists feared contamination of waterways and drinking water. The funds will preclude this from happening while improving their biosolid disposal and reclamation technology.

Federal Report Outlines Indiana's Climate Change Impacts

Global warming has been linked, for the moment, to the production of carbon dioxide, creating an ecological challenge and potential doom. Indiana is not exempt from the costs and consequences of global climate change, and a new federal report detailed some of what the Hoosier State can expect over the coming years.

The biggest negative impact expected is increased flooding, as heavier rains and shorter, milder winters (milder for Indiana, of course) mean the kinds of destructive floods familiar in the Great Plains become even more devastating. Overall, however, the longer planting seasons and higher precipitation suggests Indiana's powerful agricultural sector could in fact benefit. Migratory birds once uncommon to Indiana have begun to roost there, emigrating from the more southern states they settled in decades ago. This same habitat shift could mean new invasive plant species however, limiting agriculture's gains.

Fusion Brings Good News to Ford

Ford officials expressed their undeniable euphoria because 21 different Ford vehicles made it to the Consumer Reports' list of recommended buys. Among the vehicles are Ford Fusion, Ford Five Hundred, and Mercury Milan. In addition, Fusion was made Consumer Reports cover.

Analysts in the auto industry expect that Fusion will follow the footsteps of Taurus without becoming what it eventually became. It can be recalled that Taurus was considered one of the bread and butter vehicles of Ford. In between 1986 and 1995, you cannot imagine the automaker will hit the market without the presence of Taurus. From 1992 to 1996, the vehicle was the best selling car in the United States.

Taurus came into the market and became an instant success. The vehicle was a milestone design for Ford. Ford Taurus car parts are meticulously planned and tested to come up with a very influential vehicle.

Taurus was redesigned in 1996. Unfortunately, said redesign ranked as one of the worst redesigns in the auto history. According to enthusiasts, the makeover was goofy, flaccid and ovoid while the original Taurus was sober, taut and straightforward. Eventually, Ford decided to end the production of Taurus in October this year.

Finally, Ford has come up with a vehicle that has potentials to be the next big thing in the industry. Fusion is the vehicle that can serve as a mighty rival for Camry, Altima, Accord, and other state-of-the-art vehicles in similar segment.

Fusion is built on Ford's CD3 architecture. It measures 190.2 inches long, about 3.4 inches longer than Mazda6. Its 107.4-inch wheelbase is also longer by 2.1 inches. With regards to width, Fusion measures 72.2-inch-wide while Mazda is 2.1 inches broader.

In regard to the vehicle's rivals, they are almost exactly Fusion's size. Accord, for instance, measures 189.5 inches long, 71.5 inches wide, and rides on a 107.9-inch wheelbase. Compared to Taurus, Fusion is smaller.

Fusion is shipped out with 2 different 'Duratec' engines that are shared with the Mazda - base four and the V-6. the former is an all-aluminum 2.3-liter, DOHC four that produces 160 horsepower at 6500 rpm and 150 pound-feet of peak torque at 4000 rpm. The latter has DOHC heads, 24-valves, and generates 221 horsepower at 6250 rpm and 205 lb-ft of peak torque at 4800 rpm.
